Medicare's New GLP-1 Bridge Program: How NYC Patients Can Get Wegovy or Zepbound for $50/Month Starting July 2026

If you are a Medicare beneficiary in New York City and your doctor has recommended a GLP-1 weight loss medication, the next few months could change what you pay significantly.

Starting July 1, 2026, the Centers for Medicare & Medicaid Services (CMS) is launching the Medicare GLP-1 Bridge - a program that gives eligible Part D enrollees access to Wegovy, Zepbound (KwikPen), and Foundayo for a flat $50 per month, regardless of where you are in your Part D benefit.

At 79th Street Pharmacy on the Upper West Side, we stock all three covered medications and offer free same-day cold-chain delivery across NYC and New Jersey. Here is everything you need to know.

What Is the Medicare GLP-1 Bridge Program?

The Medicare GLP-1 Bridge is a temporary demonstration program run by CMS designed to bridge the gap while permanent GLP-1 obesity coverage policy is finalized. It runs from July 1, 2026 through December 31, 2027.

The program allows eligible Medicare beneficiaries enrolled in a Part D prescription drug plan - either a standalone PDP or a Medicare Advantage plan with drug coverage (MA-PD) - to access certain GLP-1 medications at a flat $50 monthly copay.

This is significant. Without this program, the retail price for these medications often exceeds $1,000 per month.

Which Medications Are Covered?

Three medications are covered under the Medicare GLP-1 Bridge starting July 1, 2026:

Wegovy (semaglutide)

All formulations and doses of Wegovy are covered. Wegovy is an injectable semaglutide approved specifically for chronic weight management.

Zepbound (tirzepatide) - KwikPen Only

The KwikPen formulation of Zepbound is covered. Important: The single-dose vials and single-dose pens of Zepbound are not included in the Medicare GLP-1 Bridge. If your prescription is written for a single-dose vial, speak with your prescriber about switching to the KwikPen.

Foundayo (orforglipron)

All formulations of Foundayo, the first oral GLP-1 receptor agonist for weight loss, are covered under this program.

79th Street Pharmacy stocks all three of these medications across all doses and strengths.

Who Qualifies for the $50 Copay?

To be eligible for the Medicare GLP-1 Bridge program, you must meet all of the following:

  • Enrolled in a Medicare Part D plan (standalone PDP or Medicare Advantage MA-PD) in 2026
  • Your prescriber submits a prior authorization (PA) on your behalf
  • You are prescribed the medication for chronic weight management (not diabetes)
  • You meet BMI and clinical criteria, which generally means:
    • BMI of 27 or higher plus a qualifying condition such as heart disease, prediabetes, hypertension, high cholesterol, or obstructive sleep apnea, OR
    • BMI of 30 or higher on its own

Your doctor or prescriber handles the prior authorization. If you are a patient of ours, our pharmacists can help you understand what documentation to prepare and can coordinate with your provider's office.

What the $50 Copay Does and Does Not Cover

What it covers

The $50 monthly copay gives you access to a full supply of your covered GLP-1 medication at any participating pharmacy - including 79th Street Pharmacy - for the duration of the program (July 2026 - December 2027).

Important limitations to know

  • The $50 copay does not count toward your Part D annual deductible
  • It does not count toward your $2,100 annual out-of-pocket cap under Medicare Part D
  • Low-income subsidy (Extra Help) does not apply to reduce this copay further
  • The program is temporary and runs through December 31, 2027 only

These limitations are worth knowing so there are no surprises at the pharmacy counter. If you have questions about how this fits into your specific plan, call us and we will walk through it with you.

Do I Need to Find a Special Pharmacy?

No. Pharmacies do not need to opt in or apply to participate in the Medicare GLP-1 Bridge. CMS has established a processing system that any participating Part D pharmacy - including 79th Street Pharmacy - can use to submit your claim.

What matters is that your pharmacy has the medication in stock, can process the Medicare GLP-1 Bridge claim correctly, and can get it to you safely.

At 79th Street Pharmacy, we maintain full inventory of all GLP-1 covered medications across every dose and strength, stored in temperature-monitored medical-grade refrigeration at 36-46°F.

Frequently Asked Questions

Can I use the Medicare GLP-1 Bridge if I have Medicare Advantage?

Yes. The program covers beneficiaries enrolled in Medicare Advantage plans that include Part D drug coverage (MA-PD plans), as well as standalone Part D prescription drug plans (PDPs).

What if I am already taking Wegovy or Zepbound and paying out of pocket?

If you are currently using a self-pay program - such as the $199/month intro pricing available at 79th Street Pharmacy - you may still benefit from switching to the Medicare GLP-1 Bridge. We will compare your options and tell you which one saves you more. Sometimes cash still beats insurance. We will always tell you the truth.

Is this program the same as the BALANCE Model?

No. The BALANCE Model is a separate, longer-term CMS initiative. The Medicare GLP-1 Bridge is a short-term demonstration program running through December 2027 while broader GLP-1 obesity coverage policy is developed.

What happens after December 2027?

The program is temporary. CMS has not yet announced what permanent coverage will look like after December 31, 2027. We will keep this post updated as more information becomes available.

The Bottom Line

For Medicare patients in New York City who have been priced out of GLP-1 medications, the Medicare GLP-1 Bridge is the most significant access change in years. A flat $50/month copay for Wegovy, Zepbound KwikPen, or Foundayo - starting July 1, 2026 - makes medically supervised weight loss achievable for hundreds of thousands of people who previously could not afford it.
